Administration, Health and Safety Consent

 

I/ We understand that it is my choice to attend this parish event and by participating in the activities I/ we are expected to follow the instructions provided by the organizing committee and parish volunteers at the event site..
I/ We understand and hereby agree that it is my/ our responsibility to look after my child through out the event.
I/ We acknowledge that the choice to participate in some programs brings with the individual the ASSUMPTION OF RISK and that it is my right to obtain as much information as I require, about any program or activity organized by the parish and the associated risks and hazards, including information beyond that is provided to me by the Parish.
I/ We understand that, in case of emergency medical or hospital services required for me/our child, my acceptance on this form authorizes the Parish to secure required medical advice and services as they deem necessary for my/ my child’s health and safety, and that I shall be financially responsible for such advice and services.
I/ We acknowledge that some foods may contain or come into contact with nuts, nut-based ingredients, lactose or dairy products.
I/ We acknowledge that zero tolerance is the policy of the Parish and Eparchy to ensure children’s safety. In case of any concerns, I/we will inform the designed authorities of the Parish.

In case of any possible conflicts/concerns I/ we have with the organizing committee and volunteers, I/ we will address it in a respectful and appropriate manner, and I/ we are free to contact the Parish Priest, Trustees, or an authorized/ designated person to deal with such concerns.

Consent to Post or Publish Photo/ Videos

 

St. Mother Teresa Parish, Ottawa is requesting your permission to use your personal information (i.e., Name, Grade, image, video, or any creative content prepared in the parish day) in:

 

  – Programs and activities organized in the Parish, Forane and Eparchy  .

 

 –  Publish in the parish bulletin and other digital/ printed media of the Parish/ Forane/ Eparchy

 

 –  Activities and competitions coordinated by the parish and eparchial associations and ministries.

The activities/ programs/ competitions may be presented/ published/ posted in print and/or digital media/ virtual platforms including Parish or Eparchial publications, reports, notice board, yearbooks, newsletters and online virtual platforms.

Such works may also include video recordings or display of images, where the faithful community and/or public may have access to this information and may identify you or your child.
